Don't reinvent the wheel
West's Federal Forms. This form book, while emphasizing federal procedure, is incredibly useful, even when preparing pleadings, motions, or discovery for state cases. The form may differ, and the rules may change, but the substance of the forms often translate very easily to state documents.

THE INDEX
Start in the index, at the end of the set. The index will provide a section number only (no topic). The researcher then locates the specific volume containing that section number. Just look at the spine of the books!

THE MAIN VOLUME
As you can see by the example above, the forms are very specific, and very straight forward. Simply take the verbiage that applies to your legal matter, modify it as needed, and you have your document.
