Don't reinvent the wheel
This is a very useful form book for (obviously) creating forms relevant to corporations. The forms are general in nature, but have been written with the intent to be relevant to the broadest of jurisdictions. This form book is best utilized in conjunction with a form book from your jurisdiction, or even a template of a form from the law firm in which you work.

THE INDEX
The index provides a section number that leads to a form in the main volumes. The reference number is the key to accessing the material in the main volumes.

THE MAIN VOLUME
Notice on the page above that the main volumes are broken into Chapters. Even so, the researcher does not care about the Chapter number. The section number provided in the index ignores the Chapters. The only reason for the Chapter references is to let the researcher identify the general subject matter being dealt with in that portion of the volume. As you can see on this page, Fletcher not only provides forms, but also references to other sources that may assist the researcher.
